This is a Michigan Vehicle Power of Attorney Form, it is used to create a Power of attorney or a vehicle, watercraft or mobile home. This form cannot be used to apply for an instant title unless the agent whom you are going to give the power-of-attorney is a licensed vehicle dealer. How to complete this Michigan Vehicle Power of Attorney Form?
To complete this form you must fill in the following details; the full name of the person whom you are going to appoint as your agent, and the agent’s address, the agent’s driver license number or ID card number. Do note that as your agent appointed by using this power-of-attorney he or she can sign your name to legal documents pertaining to the sale, purchase or titling of the vehicle in question.
You must also enter the following details related to the vehicle; it’s year, vehicle identification number (VIN), make and the license plate number. You must also enter the details of the owner of the vehicle as follows; name, street address, city, state, ZIP Code, date and phone number. You must also provide the owner’s Driver’s license or ID card number.
Do remember that you the owner must provide a photocopy of your valid driver license or state ID card to the agent whom you are appointing as your power of attorney.
