This is a Michigan Tax Power of Attorney Form (Form 151), it is used to authorize and individual or a company to act on your behalf in matters related to taxes. You can also use this form to designate a representative will receive copies of correspondence related to any particular a tax dispute. How to fill this Michigan Tax Power of Attorney Form (Form 151)?
In part one you must enter your details i.e. a taxpayer or debtor information like; your full name, address, FEIN, ME or TR number, your email address, your daytime phone number and fax number.
Part two deals with revocation of authority, you can use this part or revoke any other prior Power of attorney that you have created. Then in part three you must appoint a representative who will represent you in tax matters.
