In the first section you must enter the details related to you and the person whom you are appointing as your patient advocate. You must also provide the patient advocates address. As your attorney-in-fact (patient advocate) he or she is granted powers to exercise in your name and for your benefit related to; making decisions regarding your health care, custody and medical treatment.
