The form begins with the complete description of the vehicle including the vehicle identification number (VIN #), the year of the vehicle, the make, the body style of the vehicle followed by the full and legal name of the attorney-in-fact (first, middle, last) and the complete address including the city, state and the zip code.
Followed by the complete details of the principal or the original owner of the vehicle including their full and legal name (first, middle, last), drivers license number, date of birth and the complete address along with the zip code certifying that the principal is the original owner of the vehicle and holds all rights for the same and in complete sound state of mind gives the aforementioned attorney or agent the complete authority and the rights to take decisions on their behalf which shall be considered as equal to theirs in matters related only to the described vehicle.
Lastly, the document needs to be signed by the principal or the grantor for confirmation. This document also needs to be notarized by the Notary Public for it to be considered legal and the copy of the same needs to be submitted to the motor vehicle division in the state of Arizona.
